What are Fibroids?

1. Non-cancerous tumours in the muscle of the womb (uterus), also known as Myoma
2. Very common - 1/5 of women of child bearing age
3. Female hormone, estrogen, stimulates growth of fibroids

Presentation

1. Asymptomatic - discovered during routine screening
2. Increase in menstrual flow
3. Pressure symptoms on bladder or bowel
4. Low backache to sharp pain
5. Infertility problem

Conservative Management

1. Observe
2. Symptomatic treatment
3. Hormonal therapy, e.g. Progestogen, Danazol, GnRH agonist

Myomectomy
(removal of myoma / fibroid)

• Laparotomy - open surgery

• Laparoscopic - "key-hole" surgery

• Hysteroscopic resection (TCRM) - surgery through a scope in the womb for fibroid in uterine cavity (submucous fibroid)

Hysterectomy
(removal of the womb)

- Laparoscopic (MIS), e.g. LAVH (Laparoscopic assisted vaginal hysterectomy)

- Subtotal hysterectomy (removal of upper 2/3 of uterus)

- "emotional attachment to the womb"
- "Sex life"

• Hysterectomy
- Completed family
- Prevent recurrence
- Multiple fibroids
- Older women
- Associated with other disease, e.g. gynae disease CIN III
